WellCentric DC’s core approach models a CIN/ACO shared-savings strategy where we support the clinician with resources and analytics in their effort to “do the right thing”:
- Intentional recognition that we both want what is best for the person at the center of activities (the patient/member)
- Physician Practices have designated Case Managers to coordinate care with their patients (our members) on their behalf
- WellCentric DC Case Managers are integrated into the offices they are assigned to. This increases compliance with physician-endorsed recommendations and adding to their available resources for our members.

IOM made specific recommendations to HHS in July 2016 to include social risk factor scoring for Medicare and Medicaid patients. Based on the HHS recommendation and findings, 50-80% of health outcomes are impacted by social determinants (e.g. cultural competence and health literacy). WellCentric DC has incorporated Social Determinants of Health into its Case Management platform to inform the care team about factors that place the individual at risk for diseases and/or disparities in access/healthy outcomes. Both publicly available data (address, sector, environment, access to fresh food and transportation), person-specific information (socioeconomic status, education, income and social supports), and clinical information (obtained from interfaces with the physician’s EMR) are gathered with algorithms used to assign a risk-factor and influence clinical decision support. The risk-factor is used to develop the member’s treatment plan, connect the member to community resources and influence health outcomes.

WellCentric DC recognizes the power of gamification in behavior modification. Gamification uses "gaming" elements to motivate and engage people in non-game contexts such as health and wellness. Wellcentric believes that individuals are motivated by both extrinsic and intrinsic factors; however, life-long changes are sustained through intrinsic factors that modify behaviors. Gamification models adopted by WellCentric will be used in a way that supports evidence and limits unstainable changes and use of monitory incentives.
